The following two sentences in Section 2.1.1 (Attribute)
in the KMIP
specification on lines 205-207 should hopefully clarify
your first
question:

  The Attribute Index of an attribute SHALL NOT
change when other
  instances are added or deleted. For example,
if a particular
  attribute has 4 instances with Attribute Indices
0, 1, 2 and 3,
  and the instance with Attribute Index 2 is
deleted, then the
  Attribute Index of instance 3 is not changed.

I.e., the 4th instance will not be shifted and the
order is 0,1,2,4.

KMIP does not limit the number of instances of the
attributes, but
a server implementation might. For this situation,
a Result Reason
of "Index Out of Bounds" is defined (see
e.g. Section 11.14 which
lists errors for the Add Attribute operation).
