It depends on the workflow and the other capabilities of the combined package.
If there are EFSP's and they need to unpack and repack filings, then it might be good to have a third option also available for an envelope and a document enclosed so the envelope can be stripped off and the document can be put inside other envelopes routed to new destinations.
Alternatively, if the same result can be accomplished with headers that support multiple chained destinations from the beginning as with SOAP messages, then it is possible that a single combined message and document is okay.
The point is that the more flexible the standard in terms of possible necessary scenarios, the better.
