typo - ReferenceConcatentationOp

Hi there,

There appears to be one typo in the spec, that is
ReferenceConcatentationOp.

It's probably meant to be ReferenceConcatenationOp, without the 't'
after the second 'n' in Concatentation.

It appears in two places.  First on page 62 right before Table 1, and
second on page 68 in the first paragraph of Section 5.9.
