MQTT Transfer Specification PR

I've created a PR with updates toward WD04.  The specifics can be viewed / reviewed / commented on at:

This PR:

Simplifies the proposed default topic structure

Enhances the list of operating model questions

Provides initial material on the use of MQTT control packets

Updates the example messages to align with default topic structure changes
