Standard Schema

This draft has been updated: Richard Sand added an Organization entity.

It still talks about SAML attributes (rather than DSML attributes) and
it still assumes that it is necessary to specify a "Standard Schema" 
profile.
We can track these as issues.  For now, I'd prefer to defer these issues
in favor of discussing schema entities, attributes and correspondence.
