Issue: drop name attribute of <start>?

The name attribute of <start> seems to have some complex interactions with 
the combine/include mechanism.  Since it is only a syntactic shortcut, I 
wonder whether it would be better to avoid this complexity and drop the 
name attribute on <start>.
