Yes, I think this is needed.  It's not quite that simple because attributes
on annotation elements don't have this restriction.  Also a similar
restriction is needed on annotation elements.

I guess "Constraint: unqualified name" relates to the following sentence
in section 3 of the RELAX NG Specification:

"A foreign attribute is an attribute with a name whose namespace URI
is neither the empty string nor the RELAX NG namespace URI"

So to be consistent, shouldn't the non-terminal prefixedAnnotationAttributes
have a constraint saying that the attribute name must not have
the RELAX NG namespace URI?
