ISSUE 53: Contribution Extension Model

Attached is a proposal for the additional
text that needs to be added to section 10 of CD01 to describe the import
and export extension points for contributions introduced in section 11.
 As part of the proposal, the order of sections 10 and 11 should be
reversed.
