Proposed resolution to issue BINDINGS-39

Folks,

Here's my proposed resolution to issue
BINDINGS-39, as updates to cd02.  Updates are in section 6.3 and 6.4
only.

In making these changes I'm assuming
that it is valid to have a different binding on the reference/service and
the callback, and that this scenario can occur between SCA components as
well as when interacting with external applications.
