JCA binding spec documents uploaded

Folks, 

I've uploaded an updated JCA binding
specification and corresponding TA document. 

The main updates in this revision are
to apply the resolutions to BINDINGS-120, 121, 122, 123, 125 and update
the assembly reference to cd05. 

These changes cover all resolved issues.
 I would request that this revision is reviewed by the TC members
as the candidate for cd04, and the second public review draft.
