Start of C++ WD 03

I have started working on a WD 03.  I
accepted all of the changes from WD 02 related to resolved issues as well
as the conformance statement related changes we have already gone over
and concluded were correct.  I expect this will continue to evolve
as we continue the conformance statement work as well as resolve issues.
