ISSUES 44 and 45: API Details

Attached is a first pass at providing
more detail for our APIs.  At this point I am looking for feedback
on format as well as content.  And as I prepared this, a couple of
questions came up as well.  These are identified as comments.

For C, one of the key bits of this was
detailing the various error conditions and the corresponding completion
codes and reasons. 

For C++, most of the new detail deals
with return values in error/corner cases.    I have not covered
RefCountingPointer or the SCAException classes, yet.  I would like
input from others as to if the additional detail is needed for these classes.
