Issue 4 application problem

Anish,

I was looking for text on the new InvalidServiceException that was part of
issue 4 resolution.  It is present in the text at the point where
re-injection is discussed, but is missing from the API chapter where the
other exceptions are listed.  We need some text that disambiguates ISE from
SUE.   Anish, can you just fix this?
