All CAA issue resolutions to date applied in cd01-rev2

Hi,

I have updated the CAA spec with all the issue resolutions to date 
(including one from today's call). cd01-rev2 [1] [2] contains all the 
updates.

When sending new proposal or raising new issues please use the line 
numbers from cd01-rev2 PDF version.

JIRA has been updated to reflect the new status ("Applied") of the 
resolved issues as well as a pointer to the spec version.
