meetings.  Leave Dec 20

Bryan Aupperle: We should format this
document in a manner that is consistent with the other SCA specs (i.e.
use of the Code and Example styles, add captions to all examples and pseudo-schema
and the like, make references to examples and pseudo-schema and the like
explicit - remove all things like "the following ...')

of this issue.
