Issue 15 -- Syntax for Policy Attachment

See attached.  I have removed the ability to attach intents via this 
mechanism.
I cannot remember, and the minutes do not say, whether we also agreed to
remove the ability to attached WS-Policy policies and policy references.
If so, we need to remove those child elements as well.
