Core 19, document and schema

All,

	Attached are the first cut at core-19.

	There are still a number of passes that I need to go through to
check for consistency etc. However at this stage it is probably best to send
something out in time for the call tommorow.

	The schemas have been modified slightly since they were posted last
week. In redefining the AudienceRestrictionCondition I realized that not
specifying any audiences should be prohibited in a condition since that
would mean nobody could ever rely on it. If you don't want an audience
restriction don't add the condition at all.
