completed issues

I think I completed all of the changes
for the issues relating to the base spec (not the policy doc) with the
following exceptions:

- i071 - couldn't figure out how to
remove the period from Appendix titles

- i071 - couldn't figure out how to
indent the subsections for the appendix sections in the ROC

- i074 - didn't do - ran out of time.

Need to run off to the airport - perhaps
someone else could look into these?
