Modified templates from David

Attached are the "tweaks" I have made to David's contributions.

If there are no objections I will post these (along with the other Use
Cases) out to Kavi using the new format we discussed today.

Note: The templates will be in RTF format.  AFAIK RTF format is
cross-platform compatible.  If there are no objections I will assume
that the RTF format should be used for the templates.

I've also attached a blank template.   Note that the blank template
allows contributors to modify the template as needed.  Again, let me
know if there are any objections on this.
