Title: Section 4.5 editorial update

I have reworded lines 943-944 and 950-957 in the 4.5 Fault sub-section to follow-up on my previous remarks on editorial confusion.

Other editorial shortcoming: conformance with WS-I BP 1.0 (although only recommended)

is lacking the following:

We have to use a specific HTTP status code, when we generate a SOAP Fault.

BP 1.0

R1126: "An INSTANCE MUST use a "500 Internal Server Error" HTTP status code if the response message is a SOAP Fault."

I have proposed inserts to the HTTP binding section to accommodate this (see in proposed text)
