Some Minor Editorial Issues Surrounding XPath Processing

With respect to the XACML 3.0 core specification (Committee Specification 01):

In the example in Section 4.2.2, the
urn:oasis:names:tc:xacml:3.0:content-selector attribute has an attribute value
of the xpathExpression data-type that makes use of the namespace prefix "md".
The "md" prefix is undefined at this point. A namespace declaration for this
prefix needs to be added to the <AttributeValue> or one of its ancestors.

Section 7.3.7 and Appendix A.2 make use of terminology from XPath version 2.0,
namely "document node", but the "XQuery 1.0 and XPath 2.0 Data Model" is not a
normative reference.

Appendix A.3.15 refers the reader to Appendix B.3 for the details on context
nodes and namespace mappings. The relevant information is actually in Appendix
A.2.
