1.3. Must be able to represent paired codes that have been separated

Actually the remaining issues for this item seem to be the same as for the 1.8:  == A) Need to work out the mechanism to point to the opening code.  == B) Can we use the same id value for both opening and closing code, possibly using idref on closing?
