request to unsubscribe

Sorry to spam the list on this, but I have sent this message to the list
owner several times over the past few weeks, and gotten no action or
response.

I am subscribed to this list twice, as  and
. The first address is going away, and I can't replicate it
to unsubscribe automatically. I need the list manager to delete the
 address manually.
