Specification references, genericization and discretionary items

When implementing the gray-areas part of the suite configuration instance, 
I realized we will have to update the Discretionary document to use the 
same genericization being done for the test cases.

This involves changing the item/spec and item/cite constructs to be 
item/spec-citation in the discretionary items document model.

On the surface I cannot see any other changes to the discretionary document 
for genericization.  We could possibly constrain choices/choice/@value 
values, but I don't see any problem with them being unconstrained.

p.s. I haven't started this change yet as I wanted to get feedback for any 
other changes to the discretionary document.
