update to the WS-Trust profile CD04

Attached is the WS-Trust profile CD04 with format changes
made at the request of the OASIS staff. 
Since the changes were not substantive, I kept the version number
constant.  If I introduced any “substantive”
errors, please let me know.  I believe
the CD will be released for public review later this week.  Please make additional comments during
that time and encourage others to look at the standard and make comments to the
publically accessible XSPA comment list.
