BTP conf call - Four levels of operation-combination sort of thing.

As promised, a draft of the four levels of whatever they are, as discussed
in the conference call.  I've tried to cover what was said/agreed, and
included some of the discussion (filtered through my understanding).

Since the discussion was on a general phone-call and the terminology is
central, I'm sending this to the main list.
