DOCBOOK-APPS: Orphan title again

Hi all,

After much trouble I could finally reproduce the problem I had
repeatedly for my books.

Look at page 1 of attached ess.dvi for orphan section title (disappears
when lang="en") and page 2 for orphan glossary term.
