Thank you very much; it turned out that I was using an older version. 

Now I use version 1.24 (2003/03/06) and the "proportional-column-with(1)"
entries in the page header and footer have vanished.

However, in tables where I've specified colwidth="2*" an extra entry
"proportional-column-with(2)" in each row still remains and the contents of
the cells are written one upon the other.
