Resolved by moving over to docbook v5 and snapshot stylesheets.

Issue:

<table xmlns="http://docbook.org/ns/docbook" 
xmlns:xi="http://www.w3.org/2001/XInclude" border="1" floatstyle="left" 
id="d0e6">

The namespaces are being copied over,
and the 'floatstyle' attribute, which (I think)
should be converted to float='left' ?
