Proposed resolution to issue BINDINGS-148

As per the proposal for 147 for the JMS
binding, here's the web service binding proposal:

Summary: Include test assertions content
in an appendix of the test cases document; remove normative reference to
TA document, update descriptive text to reference the new appendix; move
mapping section from body of document to appendix.

One comment here is that the test assertions
still have comments which seem to indicate there are open questions on
them, we need to clean up those comments before the next review.
