additional changes for 10 draft

As noted in my email yesterday, there were some issues that got 
bypassed in the ftf, among these some reasonably manageable ones in 
section 2.2.1.  The attached Word document has the changes and these 
can be identified as comments KJL23 to KJL30.  In the attached Excel 
issues list, I modified the comments in lines 34-60.
