New Issue RMD: Remove inheritance from RMD documents use cut/copy/paste

WS-RMD currently uses an inheritance mechanism to logically combine RMD
documents.  This makes the processing
model for clients working with RMD documents onerous.
Should we simplify the 'inheritance' model to be the same as WSDL1.1
documents (ie, cut/copy/paste)?
