Unfortunately all the current implementations
look like they basically were to test the soundness of the spec rather
than clearly pointing at web stacks incorporating support.
Your second reference was interesting
as the only comment about MTOM was relative to it extending SOAP1.2 to
include attachment support. Is this the time to introduce SOAP1.2 bindings
and add the MTOM support as extending those bindings? Perhaps the WSDL
SC should take this up and bring a recommendation to the TC.

Note that these are all SOAP 1.2 so we may
wish to define two new protocol bindings: _SOAP1dot2
and _SOAP1dot2withMTOM
for WSRP 2.0.
Axis2 and Microsoft's Indigo have announced
planned support for MTOM/XOP and here
is a reference which suggests JAX-RPC 2.0 will include MTOM:

No idea if these and other implementations
will interop from day one.
